Scope of This Policy

This Privacy Policy describes how Wyn Chiropractic & Rehabilitation PLLC, doing business as ReVibe Chiropractic and Rehabilitation ("the clinic," "we," "us," or "our"), collects, uses, and protects information through our website at www.reviberehabilitation.com and through the email, telephone, and text message channels we use to communicate with patients and prospective patients.

Protected health information created or received in the course of your care is governed by our Notice of Privacy Practices, available at www.reviberehabilitation.com/notice-of-privacy-practices, which describes your rights under the Health Insurance Portability and Accountability Act (HIPAA) and the Minnesota Health Records Act. Where this Privacy Policy and the Notice of Privacy Practices differ with respect to protected health information, the Notice of Privacy Practices controls.

Information We Collect

Information you provide directly. When you contact us through this website, request an appointment, or communicate with us by phone, email, or text message, we collect the information you choose to give us. This typically includes your name, phone number, email address, and any details you include in your message.

Information collected through scheduling and patient forms. Online scheduling, intake forms, and patient records are hosted in our electronic health record and practice management system, which is operated by a third party service provider under a Business Associate Agreement. That provider maintains its own privacy policy governing its platform. A link to the scheduling system is available on this website.

Information submitted by referring providers. Health care providers may refer patients to us through a referral form on this website. That form collects the referring provider's practice name, clinician name, phone number, and email address, and information about the patient being referred, including the patient's name, phone number, and the reason for referral. The referral form is hosted by a third party form provider that has agreed to protect health information under a Business Associate Agreement, and submissions are stored in our secure environment rather than on this website. Information about a referred patient is protected health information and is handled under our Notice of Privacy Practices from the moment we receive it.

Information collected automatically. Our website platform records standard technical information when you visit, including IP address, browser type, device type, referring page, and pages viewed. Third party analytics and advertising providers may also collect this information on our behalf, on this website and on our online scheduling pages. This information is used to operate and secure the site, to understand general traffic patterns, and to measure how our advertising performs.

Cookies and analytics. Our website uses cookies and similar technologies that are necessary for the site to function, and analytics cookies and tags, which may include Google Analytics or comparable services, to measure traffic and understand how visitors use the site. Our online scheduling platform may also use analytics tools on its own pages. Most browsers allow you to refuse or delete cookies, and Google offers a browser add-on to opt out of Google Analytics. Blocking cookies may affect how parts of the site work.

Advertising. We may use advertising and conversion tracking technologies, including services offered by Google and comparable advertising platforms, to measure how our advertising performs and to show our ads to people who have visited our website. We do not use these technologies on pages where you enter health information, and we do not use them to target advertising based on the care you receive here. We do not sell personal information, and we do not disclose patient information for marketing purposes without your written authorization.

How We Use Information

We use the information described above to:

  • Schedule, confirm, reschedule, and remind you about appointments

  • Provide and coordinate your care

  • Bill for services and communicate about your account

  • Operate, maintain, secure, and improve the website

  • Measure and improve the performance of our advertising

  • Comply with legal and professional obligations

Text Messaging (SMS)

No sale or sharing of mobile information. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes at any time. ReVibe Chiropractic and Rehabilitation does not sell, rent, share, release, or trade mobile phone numbers or SMS consent data to any third party, affiliate, or lead generator. All categories of information described in this policy exclude text messaging originator opt-in data and consent, and that information is not shared with any third parties. Mobile phone numbers and text message opt-in information are shared only with the messaging service providers that transmit messages on our behalf, and only for the purpose of delivering those messages. Those providers are contractually prohibited from using the information for any other purpose.

Numbers provided by someone else. We do not enroll a phone number in our text message program when it is submitted to us by a third party rather than by the person who uses that number. If a referring provider gives us a patient's phone number, we reach out to that patient directly. We do not send that patient text messages unless and until the patient asks us to.

How we obtain consent. We obtain consent to contact you by text message when you provide your mobile number and agree to our communication terms on our patient intake forms, when you provide your mobile number and consent verbally or in writing at our clinic, when you send a text message to our clinic number, or when you request a scheduling link through our phone system.

What we send. Appointment reminders and confirmations, scheduling and account notifications, billing and benefit notices, recommendations to return for follow-up or a recheck, replies to messages you send us, scheduling links you request, and an occasional request to review our clinic. Review requests are sent infrequently. You can decline them at any time by replying STOP or by telling us.

We do not send promotional or advertising text messages. If we add promotional messaging in the future, we will obtain your separate consent first.

Consent is not a condition of care. Agreeing to receive text messages is not required in order to become a patient or to receive treatment.

How to stop. Reply STOP to any text message to stop receiving them. You may also tell us in any other reasonable way, including by phone, email, or in person, and we will honor that request. We do not require any particular method, and we act on opt-out requests promptly and in no case later than ten business days after receipt. Reply HELP for assistance. Message and data rates may apply. Message frequency may vary.

How We Share Information

We share information only as follows:

Service providers. We use vendors to run the practice, including our electronic health record and scheduling platform, telephone and messaging providers, our website host, website analytics and advertising measurement providers, payment processors, and billing and clearinghouse services. Vendors that handle protected health information do so under a Business Associate Agreement and may use the information only to perform services for us.

Administrative services. ReVibe Fitness and Wellness, Inc. is a separate company that shares our location and licenses us the ReVibe name. It provides certain administrative and operational services, including scheduling and reception, under a management services arrangement and a Business Associate Agreement. Its personnel access patient information only as necessary to perform those functions.

Treatment, payment, and health care operations. Health information is used and disclosed as described in our Notice of Privacy Practices.

Legal requirements. We disclose information when required by law, in response to lawful process, or to protect the rights, safety, or property of patients, staff, or the public.

We do not sell personal information. We do not sell, rent, or trade personal information, and we do not disclose patient information for marketing purposes without written authorization.

Security

We maintain physical, technical, and administrative safeguards designed to protect the information we hold. Patient records are kept in a secure electronic health record system with access limited to authorized personnel. Devices and systems that access patient data are password protected and encrypted where applicable.

Email and text message are not fully secure channels. Please do not send sensitive health information through the website contact form or by unencrypted email. If you prefer a different method of communication, tell us and we will accommodate any reasonable request.

Retention

We retain patient records for the period required by applicable federal and state law and professional standards, and we retain other information for as long as needed for the purposes described in this policy. Website contact form submissions and general inquiries are retained only as long as needed to respond and to maintain a record of the request.

Mobile phone numbers are retained as part of your patient or contact record. If you opt out of text messages, we keep a record of your opt-out indefinitely so that we do not message you again. You may ask us to delete information you submitted through this website or through our messaging program, and we will do so except where a record must be retained under our legal, professional, or record retention obligations. Children

This website is intended for adults. We do not knowingly collect personal information from children under 13 through this website. Care for patients under 18 is arranged and consented to by a parent or legal guardian.

Your Choices

  • You may stop text messages at any time by replying STOP or by telling us in any reasonable way.

  • You may unsubscribe from email notifications using the link in the message or by contacting us.

  • You may ask us to contact you by a specific method or at a specific address, and we will accommodate reasonable requests.

  • Patients have additional rights regarding their health information, including the right to access, amend, and restrict its use. Those rights are described in our Notice of Privacy Practices.

Third Party Websites

Our website links to third party services, including our scheduling and patient portal. Those services operate under their own terms and privacy policies, and this Privacy Policy does not apply to them.

Changes to This Policy

We may update this Privacy Policy. The current version is always posted at www.reviberehabilitation.com/privacy-policy with the effective date at the top. Material changes affecting protected health information will also be reflected in our Notice of Privacy Practices.
