Agreement
These Terms and Conditions govern your use of www.reviberehabilitation.com and your participation in the email and text message communications described below. By using this website or by providing us with your phone number or email address, you agree to these terms.
These terms do not replace the consent, financial responsibility, and cancellation agreements you sign as a patient. If anything here conflicts with the intake forms you have signed, the signed intake forms control. Protected health information is governed by our Notice of Privacy Practices, available at www.reviberehabilitation.com/notice-of-privacy-practices.
No Medical Advice, No Provider Relationship
The content on this website is general information about chiropractic and musculoskeletal care. It is not medical advice, it is not a diagnosis, and it is not a substitute for evaluation by a licensed clinician. Using this website, submitting a form, or requesting an appointment does not create a provider and patient relationship. That relationship begins when you are accepted as a patient and evaluated in our clinic.

Do not use this website, email, or text message to report an emergency or an urgent change in your condition. These channels are not monitored continuously and are not appropriate for urgent clinical matters.
Text Message (SMS) Program
By providing your mobile number to ReVibe Chiropractic and Rehabilitation, you consent to receive text messages from us at that number as described below.
Program Description
ReVibe Chiropractic and Rehabilitation sends SMS text messages to patients and prospective patients about their care and their account. Messages include appointment reminders and confirmations, scheduling and account notifications, billing and benefit notices, recommendations to return for follow-up or a recheck, replies to messages you send us, scheduling links you request through our phone system, and an occasional request to review our clinic. Review requests are sent infrequently. You can decline them at any time by replying STOP or by telling us.
We do not send promotional or advertising text messages through this program. If we add promotional messaging in the future, we will obtain your separate consent first.
How You Opt In
You may opt in by providing your mobile number and agreeing to our communication terms on our patient intake forms, by providing your mobile number and consenting verbally or in writing at our clinic, by sending a text message to our clinic number, or by requesting a scheduling link through our phone system.
Consent is not a condition of care or purchase. You are not required to agree to text messages in order to become a patient, to schedule, or to receive treatment.
Frequency and Cost
Message frequency. Message frequency may vary.
Cost. Message and data rates may apply for messages sent to you from us and to us from you. If you have questions about your text plan or data plan, contact your wireless provider.
How to Stop
You can cancel the text message service at any time by replying STOP to any message from us. After you send STOP, we will send one confirmation message and you will not receive further text messages from us. You may also stop messages by telling us in any other reasonable way, including by phone at (952) 866-9010, by email at info@reviberehabilitation.com, or in person. Stopping text messages does not affect your care. If you later want to resume text messages, tell us and we will re-enroll your number.

Carriers, Delivery, and Privacy
Carrier terms. Carriers are not liable for delayed or undelivered messages.
Supported carriers. Message delivery is subject to your carrier's network and is not guaranteed.
Privacy. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes at any time. We do not sell, rent, share, release, or trade mobile phone numbers or text message consent data to any third party, affiliate, or lead generator. Email and Telephone Communications
We may also contact you by email and telephone, including through automated technology, using the contact information you provide, for the same purposes described above. Email and text message are not fully secure. You may ask us at any time to use a different method or to stop using one.
Scheduling, Patient Portal, and Third Party Services
Online scheduling, intake forms, and the patient portal are provided through a third party platform, reachable from this website. Your use of that platform is subject to its own terms and privacy policy. Requesting an appointment online is a request, not a confirmed booking, until we confirm it.
This website may link to other third party sites. We do not control those sites and are not responsible for their content, terms, or privacy practices.
Appointments, Cancellation, and Payment
Appointment times are reserved for you individually. We ask for advance notice to cancel or reschedule, in the amount stated in the patient intake agreement. A cancellation fee may be charged for late cancellations and missed appointments. The current amount is disclosed in the patient intake agreement you sign before your first visit, and is subject to change with reasonable notice. This fee cannot be billed to insurance and is the patient's responsibility.
Copays and self-pay charges are due at the time of service. Balances determined after a claim is processed are due on receipt of statement. Benefit verifications and cost estimates are a courtesy based on information from your insurer and are not a guarantee of coverage or of your final responsibility.

Acceptable Use
You agree not to use this website to transmit unlawful, harassing, or harmful content, to attempt to gain unauthorized access to any system, or to interfere with the operation of the site. Please do not submit detailed health information through the website contact form.
Intellectual Property
The content, layout, text, images, and marks on this website, including the ReVibe name and logo, are owned by or licensed to us and may not be copied, reproduced, or used without permission.
Disclaimers and Limitation of Liability
This website is provided on an as is and as available basis. We do not warrant that the site will be uninterrupted, error free, or free of harmful components, and we do not warrant that any information on the site is complete or current.
To the fullest extent permitted by Minnesota law, we are not liable for indirect, incidental, or consequential damages arising from your use of this website or from delayed or undelivered electronic messages. Nothing in these terms limits our obligations or your rights with respect to the professional care we provide, or any liability that cannot be limited by law.
Governing Law
These terms are governed by the laws of the State of Minnesota, without regard to conflict of law principles. Any dispute arising from this website or these terms will be brought in the state or federal courts located in Minnesota.
Severability and Waiver
If any provision of these Terms and Conditions is held to be invalid or unenforceable, that provision will be limited or removed to the minimum extent necessary and the remaining provisions will remain in full force and effect. Our failure to enforce any provision is not a waiver of our right to enforce it later.
Changes
We may update these Terms and Conditions. The current version is posted at www.reviberehabilitation.com/terms-and-conditions with the effective date at the top.
